Corporate Care Management

CCM Chronic Condition Case Management focuses on high-risk individuals that present with one or more chronic diseases such as Diabetes, Hypertension, Coronary Artery Disease, Heart Failure and Chronic Kidney Disease. Corporate Care Management uses an opt out model, assuring every member is welcomed into the program. Board-certified Nurses evaluate members’ treatment plans and benefit coverage before providing education and guidance to steer members toward the best care options. Registered Nurses deliver individualized, one-on-one assistance to empower members to understand their diagnoses, incorporate self-management techniques into their daily living and align themselves with their healthcare provider's treatment regimen. By providing disease education, promoting self-management of conditions, and reinforcing the provider’s plan of care, CCM assists in management of the members health.
