• Dental PPO: financial incentives and negotiated fees used to encourage members to use network dentists, while also allowing members the freedom to see providers outside the network.
• Dental TRP: encourage members to utilize lower-cost in-network care through our dental PPO network while still providing similar coverage levels for nonparticipating providers.
• Dental HMO/Prepaid: best suited for members seeking the lowest combination of premiums and out-of-pocket expenses, offering an all-copay, no-deductible, no-annual-maximum, with savings of 40-60%.
• Dental Preventive Plus: cost-effective design providing complete coverage on in-network preventive care, coverage for teledentistry services, and discounts on other dental services.
In addition, our dental PPO and TRP plans provide expanded coverage, including:
• Enhanced Preventive Care: three preventive cleanings or four total periodontal cleanings, space maintainers for children, and oral cancer screenings
• Extended Annual Maximum: 30% coinsurance paid by Humana for services after members meet their annual maximum.

• All implant-supported prosthodontics (crowns and dentures) covered without the implant rider.
